Every day we guide and support patients and their families through the realities of neuro-disabilities. And, because disability impacts emotionally as well as physically on the patients and those people close to them, we offer support every step of the way, helping people adjust to life with disability, and plan for a positive future.

How we help 
We provide a dedicated social worker to each rehabilitation patient and their family from day one through to discharge. Talking through every aspect of their immediate, short and long-term care, we help families plan for the future. From practical information such as how the team of neuro-rehabilitation specialists plans and reviews their work, to coordinating regular meetings, we help families and patients adjust to life with disability by making the journey as smooth and well informed as possible.

We do this by helping families:

- Get involved
Encouraging them to actively participate in a patient’s care pathway, helping us incorporate their interests into a truly holistic rehabilitation plan.
- Stay updated
Co-ordinating regular meetings with the rehabilitation team and carers to discuss progress.
- Cope
Supporting people emotionally and practically, suggesting ways to get involved in care and activities they can enjoy as a family.
- Make practical plans
Offering guidance on everything from statutory entitlements, Court of Protection and housing needs, to childcare and care options for the patient, whether that means day care, at-home support, or long-term residential or nursing care.
