Transitional Rehabilitation Service

There is something extraordinary about seeing a person regain enough independence to return home. We specialise in helping people rebuild the skills and confidence they need to achieve this, with particular expertise in supporting people who would otherwise struggle to make the transition, including those with physical, cognitive, behavioural, and communication difficulties.

How we help
Based in a homely environment which closely reflects community life, we help people with brain injury to work on the skills they will need to make the transition back to life beyond hospital, improving their ability to carry out everyday tasks and preparing them for a return to community living.

Each person who stays with us benefits from:

- Continuity of care
Individual rehabilitation does not just take place in therapy sessions, but continues throughout the day, supported by our on-site rehabilitation staff. What’s more, we ensure they can always have access to the RHN’s full rehabilitation services and the local community.
- Re-learning everyday skills
From getting on the bus and shopping for groceries, to personal safety and returning to work, we teach residents the skills they need to manage their new lifestyle successfully.
- Private and community space
A room and bathroom of their own, with shared kitchen and recreational areas. THere are three independent flats available.
- Future planning
Working closely with each resident’s local community services to ensure their current and future care needs will be met.
