The annual survey helps us to understand what we are doing well and what we can improve.

The survey is currently open to all patients/residents and their families and will remain open for the next six weeks.

Where addresses are available, forms will be sent by post to all next of kin. Forms will also be available

• on wards
• at reception
• from the Patient Safety & Quality Team
